Phaco Training: Sub Tenon’s Block

Sub Tenon’s block is another very convenient alternative that was once tried out almost 100% of patients in Aravind Eye Care System

1)after lid speculum is inserted..I put paracaine eye drops …then gave the betadine wash

2)I take about 5 ml of lignocaine without hylase in a syringe and put visco canula on it.

3)Hold the conjunctiva & subtenons tightly & lift up.

4)with the vannas scissors make a small nick & enter subtenons space.

5) insert the cannula gently & go along the curve of the globe for half the length of the cannula & gently push the plunger.u may ask the patient to look down

6) Give pressure for a minute

7)the action is instantaneous & u can just proceed with the surgery

8)The chemosis is minimal…but if u inject sub conjunctivally…then lots of chemosis,but still u can proceed with the surgery…

It saves lot of lignocaine and your hylase too

2 ml is enough.
